Seasons may change and you may need to adjust your policy from time to time, and Leland Smith Insurance will be with you every step of the way.

Policy change forms are available to download, print, and complete. Select the applicable form and submit completed forms by dropping off the printed form at one of our offices, faxing to (419) 238-7890, or uploading the completed policy change form below.

  • Request changes to your policy 24 hours a day, 7 days per week at your convenience by submitting the completed form below. We will process your request within 24 business hours and send confirmation of the change.

  • Max. file size: 64 MB.

Please remember that no coverage is ever considered bound until you have received either a written or verbal confirmation from a Licensed Agent in our office.
